330 Federally Qualified Health Center Expansion Opportunities

 

  • Telehealth Network Grant Program TNGP grants demonstrate how telehealth networks improve healthcare services to: (a) expand access to, coordinate, and improve the quality of health care services; (b) improve and expand the training of health care providers; and/or (c) expand and improve the quality of health information available to health care providers, patients, and their families.

    Eligibility: Non-profit or public organizations with a demonstrable successful track record in implementing telehealth technology. Programs that serve rural communities, although grantees may be located in urban or rural areas.

  • Service Expansion for Enabling Services for Special Populations Enabling services may include but are not limited to: case management, services to assist the health center's patients gain financial support for health and social services, outreach, transportation and interpretive services; and education of patients and the community regarding the availability and appropriae use of health services.
