FQHC Needs Assessment 

Demonstrating a high degree of need is the first step in applying for New Access Point (NAP) funding, and existing health centers are required to periodically document the healthcare needs of their community. NAP applicants must complete the Form 9: Need for Assistance Worksheet and are scored on specific indicators using predetermined benchmarks. This document also serves as a useful guide for FQHCs to follow in determining what indicators to focus on.  HRSA developed a Data Resource Guide designed to assist applicants with completing the Need for Assistance Worksheet.

The Accountable Care Organization (ACO) Quality Explorer: Data for 211 ACOs participating in the Medicare Shared Saving Program (MSSP). Allows visualization of 33 quality metrics across 5 domains, compared against CMS benchmarks.
