Health Disparities Collaboratives Cardiovascular Disease Mini-Grant Opportunity
Through a partnership developed between the Florida Association of Community Health Centers and the Florida Department of Health Heart Disease and Stroke Prevention Program, health centers will be financially assisted with spreading the concepts of the Health Disparities Collaboratives and the Care Model further within the delivery care system.
Up to $10,000 will be provided through this mini-grant opportunity and will be used by particpating community health centers to support the spread of the Health Disparities Collaboratives to Cardiovascular Disease.
