Each year at our annual summer meeting FACHC recognizes individuals and organizations that have made significant contributions to increasing access to healthcare and reducing health disparities in Florida. The nominees and award winners are publicly acknowledged among their peersin the FQHC community and wider network. The FACHC awards are an opportunity to acknowledge the hard work and dedication of all those involved in bringing high quality, affordable healthcare to Floridians. Below is a description of each award, followed by a link to the nomination forms. The deadline for nominations is June 1, 2011.
To annually recognize a community or migrant health center that demonstrates exemplary effectiveness in the promotion of health or prevention or reduction of disease among the population it services and to create wider public attention to the services of community and migrant health centers and their impact on improved health status of the community and especially the health underserved.
To recognize private individuals, civic or religious groups who have advanced health services to the health underserved by financial, material or voluntary services to a community or migrant health organization and to create public awareness and support for private participation in community health care.
This Award recognizes an individual whose commitment and contribution to the improvement of the health, education, and social well being of all individuals, especially those less fortunate, and whose advocacy for those individuals with special emphasis on the migratory agricultural workers in Florida, will be long remembered.
This award will be presented to the individual who has contributed most towards the conscience, goals, missions or recognition of the Florida Council of Primary Care Centers, Inc., and its members. This annual presentation can be awarded internally or externally.
To recognize an employee, organization/center or board member for their outstanding leadership contributions (s). An individual or organization who exemplifies visionary leadership in the local, regional, state, or national health center movement, challenges the status quo to create new programs and services to meet the changing health care environment and collaborates to eliminate racial and ethnic disparities in health outcomes.
