Senior Physician (Agape Community Health Center; (South) Jacksonville, FL)

Job Type: Full-Time

Position Description:
This is a position for a senior physician with experience in family practice. Physicians with training in family medicine, internal medicine, or equivalent will be considered. The position will reports directly to the Chief Medical Officer. Full time incumbents will be eligible for the employment benefits of the state of Florida while enjoying working in an enriching clinical and educational environment. Preferences are given to candidates with Florida license, board certification/eligibility, and/or experience, competency in more than one language, and skills in using electronic medical records and computers.

Primary responsibilities include but are not limited to:

  • Performing histories, physicals, and prescribing a diagnostic, education, preventive and /or treatment plan within the standards of accepted medical practice
  • May order laboratory, perform or order diagnostic procedures, and make appropriate referrals as necessary
  • Coordinating continuity of medical care for patients referred from other agencies
  • Participate in quality assurance and quality improvement projects
  • Maintaining patients’ records
  • Assist the Chief Medical Officer in implementing the clinical policies and procedures/process
  • May be required to supervise clinical staff in the clinic, prepare reports and complete forms
  • May be required to take after-hours calls and perform emergency duty before, during and/or beyond normal work hours or days
  • Attend medical and administrative meetings, assignments and coverage for other clinics within the DCHD clinic system as needed
  • Preparing and giving presentations, performing chart reviews and duties related to various committees
  • Other related duties as required

Advance Registered Nurse Practitioner (Agape Community Health Center; Jacksonville, FL)

Job Type: Full-Time

Position Description:
This is a licensed, professional, Advanced Registered Nurse Practitioner which performs patient care and medical decision-making autonomy within established clinical parameters and protocols. Will provide a broad range of physical assessments, diagnostic, therapeutic and preventive services.

Primary responsibilities include but are not limited to:

  • Perform basic physical examinations of patients utilizing various techniques such as observation, palpation, percussion, and auscultation
  • Identify and evaluate illnesses and injuries
  • Treat common illnesses and monitor the progress of acute illnesses
  • Compile and evaluate patient medical histories, physical findings and, if necessary, initiates further diagnostic laboratory tests
  • Initiate treatment under a physician’s supervision or, within the limits of established protocols, make diagnosis and choose, initiate, and modify selected therapies or treatments
  • Facilitate prescriptions for standard drugs for patients following consultation with, and the approval of, a physician - or in accordance with established protocols
  • Perform emergency preparedness operations, including, but not limited to, Special Needs Shelters, Red Cross Shelters or other emergency related functions
  • Evening and weekend assignments may be required
  • May be required to perform duties and responsibilities at other Duval County Health Department sites, as necessary
  • Other related duties as required

  • Must possess willingness to work extended, eveneing, weekend or emergency hours, as sometimes required for extending patient care and/or participation in community health events

Position Description:
The Health Care Center for the Homeless, Inc., dba Orange Blossom Family Health Center, an Equal Opportunity Employer located in Orlando, FL, is currently accepting applications for an experienced Family Practice Florida-licensed Advanced Registered Nurse Practitioner to provide quality, comprehensive and culturally sensitive primary medical care to the homeless and medically underserved population in Orange, Seminole and Osceola counties. Primary responsibilities include but are not limited to:

  • Promote good health and prevent illnesses by educating patients and their families on wellness, prevention and early detection, and by providing adequate materials and resources for consent and self-empowerment
  • Diagnose & treat diseases using evidence-based medicine while operating within the scope of practice and standards of Patient-Centered Medical Home
  • Determine the level of urgency follow-up and which referrals/consultants are needed based on appropriate examination and patient needs. The medical director should be consulted as needed for final medical decisions
  • Document all patient encounters (phone, verbal, email) in the Electronic Health Record (EHR)
  • Review and address patient’s medical records, labs, and refill requests on a daily basis
  • Consult with the medical director & colleagues for coverage when on vacation
  • Maintain confidentiality of all patient information according to federal and state guidelines and regulations
  • Take an active role in the HCCH financial viability by practicing cost-effective healthcare that is medically necessary and appropriate
  • Participate in peer review and quality improvement related activities as delineated in the center’s Clinical Quality Improvement (CQI) plan
  • Observe and reinforce all of HCCH policies and procedures maintaining excellent medical and ethical standards
  • Serve as a positive role model showing respect, compassion, integrity and timely responsiveness to HCCH patients, colleagues, and staff
  • Educate and support the HCCH clinical and non-clinical staff when needed
  • Serve as a preceptor to different professional schools (UCF, USF, etc.)
  • Collaborate with the HCCH administrators, staff, and providers (Physicians, Nurse Practitioners, Physician Assistants, Behavioral Health Therapist, etc)

RN Transitional Care Coordinator (Health Care Center for the Homeless, Inc., Orlando, Florida)

Position Description:
The Health Care Center for the Homeless, Inc., dba Orange Blossom Family Health Center, an Equal Opportunity Employer located in Orlando, FL, is currently seeking an experienced Registered Nurse to fill the position of Transitional Care Coordinator to track and monitor homeless and uninsured patients who are discharged from hospitals and transitioning into the outpatient setting. The qualified candidate must be skillful in collaborating with physicians to design care plans that focus on increasing patients' and caregivers' abilities to manage health conditions in the outpatient setting as well as to minimize frequent emergency room and inpatient hospital admission while improving health outcomes. In addition, the selected individual will participate in the Continuous Quality Improvement process; assist with maintaining the Patient Centered Medical Home as well as assist with achieving and maintaining the Center's accreditation. The chosen applicant must possess excellent computer and communication skills as well as exceptional personal skills and expertise in providing culturally sensitive primary medical care to the homeless and indigent individuals. This position is fulltime, qualifying for our excellent benefit package and reports to the Clinical Coordinator.

Primary responsibilities include but are not limited to:

  • Coordination of medical care following hospital discharge
  • Assessment of home or homeless conditions and development of plans of care to ensure optimal success in an outpatient setting

  • Providing patient education to improve self-care management

  • Understanding of the health care system and its component parts including sites of care, delivery models and the roles of various providers and health care professionals
  • Ability to manage patients with multiple co-morbidities, complexities and diverse needs
  • Comfort working in an independent role
  • Experience in providing care in a hospital and outpatient setting
  • Possess the qualities of flexibility and adaptability, assertiveness, maturity and confidence
